At the Health Point

What happens at a visit.

Each visit follows the same structured screening, recorded against the person's health profile.

Vital health monitoring
Temperature, pulse, heart rate, blood pressure, blood oxygen and ECG
Cardiovascular health
Blood pressure, heart rate, ECG and risk assessment
Chronic disease
Identification and follow-up of high-risk individuals
Specialist care
Teleconsultation and referral support

The care journey

From first screening to follow-up.

No one has to find the next step alone. This is the proposed design.

  1. 01

    Outreach

Health AgentExplains the programme at community meetings and home visits.

PlatformTracks which households have been reached.

  2. 02

    Registration

Health AgentRegisters each person, with their consent.

PlatformCreates a secure record, designed to link to ABHA.

  3. 03

    Health profile

Health AgentRecords history, medicines and basic measurements.

PlatformBuilds a digital health profile that stays with the person.

  4. 04

    Screening

Health AgentMeasures temperature, pulse, blood pressure, blood oxygen and ECG.

PlatformRecords every reading against the person's profile.

  5. 05

    Risk identification

Health AgentExplains results in plain language.

PlatformFlags readings that need a doctor. The AI flags; a doctor decides.

  6. 06

    Consultation or referral

Health AgentConnects the person to a doctor by video.

PlatformRoutes to a specialist, an empanelled lab or a Jan Aushadhi Kendra when needed.

  7. 07

    Follow-up

Health AgentChecks in on schedule and repeats screening.

PlatformSchedules follow-ups and alerts the agent when one is missed.

Health Agents

Local health jobs, built on trust.

Each block of 100 families has one or two Health Agents, drawn from self-help groups, gram sevaks and anganwadi workers.

They are trained and certified to screen, explain results and follow up. They are known to the families they serve.

At full scale, the norm of 1–2 agents per block implies about 20,000 to 40,000 Health Agents. This is derived from the proposal, not a hiring commitment.

For citizens

What it means for you.

  • What happens

A Health Agent from your area checks your blood pressure, pulse, oxygen, temperature and heart. It takes a few minutes.

  • If something needs attention

You can speak to a doctor by video at the Health Point. If you need tests or medicines, we guide you to the nearest lab or Jan Aushadhi Kendra.

  • Your information

We ask your permission before creating your record, and we protect it under Indian data protection law.

Next steps

Start with a conversation.

Three steps, and no commitment until the plan is agreed.

  1. 01

    Briefing

A 30-minute walk-through of the model, the phases and the safeguards.

  2. 02

    Scoping

We work with your officials on geography, numbers and links to existing schemes.

  3. 03

    Phased plan

A Phase 1 plan with agreed phase-gate criteria, before any commitment.
