One model, built to travel.
Four parts, one care journey, and technology that keeps a doctor in every decision. This page describes the proposed design.
The model
Four parts, one system.
Built to work wherever people already gather. It starts in villages.
Health Point
A screening station where people already gather. In villages, one for every 100 families.
Health Agent
A trained local health worker, drawn from self-help groups, gram sevaks and anganwadi workers.
Clinician network
Doctors and a specialist panel at a Health Command Centre, reached by teleconsultation.
Platform
One health record per person, an AI risk engine that flags concerns for doctors, and dashboards for officials.
How the parts connect
From a doorstep to a doctor, and back.
The proposed design links every visit to a record, a doctor and a next step.
01Health Agent
Household
The agent explains the programme and registers each person, with consent.
02Health Agent
Health Point
One device set records vitals and ECG against the person's profile.
03Platform
Record and risk engine
Readings are stored, and flagged for a doctor when they need attention.
04Doctor
Health Command Centre
A doctor reviews the case and consults by video.
05Onward care
Referral
When needed, the person is routed to:
- A specialist
- An empanelled lab
- A Jan Aushadhi Kendra
Follow-up: the platform schedules the next check and alerts the agent if it is missed.
The care journey
From first screening to follow-up.
No one has to find the next step alone. This is the proposed design.
- 01
Outreach
Health AgentExplains the programme at community meetings and home visits.
PlatformTracks which households have been reached.
- 02
Registration
Health AgentRegisters each person, with their consent.
PlatformCreates a secure record, designed to link to ABHA.
- 03
Health profile
Health AgentRecords history, medicines and basic measurements.
PlatformBuilds a digital health profile that stays with the person.
- 04
Screening
Health AgentMeasures temperature, pulse, blood pressure, blood oxygen and ECG.
PlatformRecords every reading against the person's profile.
- 05
Risk identification
Health AgentExplains results in plain language.
PlatformFlags readings that need a doctor. The AI flags; a doctor decides.
- 06
Consultation or referral
Health AgentConnects the person to a doctor by video.
PlatformRoutes to a specialist, an empanelled lab or a Jan Aushadhi Kendra when needed.
- 07
Follow-up
Health AgentChecks in on schedule and repeats screening.
PlatformSchedules follow-ups and alerts the agent when one is missed.
AI with a doctor in the loop
The AI flags. A doctor decides.
The risk engine changes the order in which doctors see people. It does not change who makes the decision.
01Health Point
Reading
Vitals and ECG are recorded at the Health Point.
02Risk engine
Prioritise
The engine flags readings that need attention and orders the doctor's queue.
03Doctor
Review
A qualified doctor reviews every alert, with vitals, trends, ECG and history.
04Doctor
Decide
The doctor chooses the next step:
- Teleconsultation
- Referral
- Routine follow-up
05Report
Count
Every reviewed alert is counted in the phase report.
Technology
What makes it work, part by part.
At the Health Point
One device set per Health Point measures temperature, pulse, heart rate, blood pressure, blood oxygen and ECG.
Teleconsultation
A tablet application connects each Health Point to a doctor, under India's Telemedicine Practice Guidelines.
Health Command Centre
Doctors and a specialist panel work from a doctor dashboard showing the queue, vitals, trend charts, ECG traces and prior consultations.
AI with a doctor in the loop
The risk engine prioritises who a doctor should see first. It does not diagnose, and every alert is reviewed by a qualified doctor.
Diagnostics and medicines
Empanelled labs serve clusters of Health Points, and prescriptions link to Jan Aushadhi Kendras for affordable generic medicines.
One platform
A single platform holds health records, risk identification, teleconsultation, referral tracking, dashboards and impact reporting.
Next steps
Start with a conversation.
Three steps, and no commitment until the plan is agreed.
- 01
Briefing
A 30-minute walk-through of the model, the phases and the safeguards.
- 02
Scoping
We work with your officials on geography, numbers and links to existing schemes.
- 03
Phased plan
A Phase 1 plan with agreed phase-gate criteria, before any commitment.
